Check the contents of the vault.ini file, if it isn't anything in it or it still saves by STEAM ID, I'll bet you have a misconfiguration somewhere.

And how about your MySQL server version? Debian Sarge carries 2 of them 4.0 and 4.1 so it matters.

You can always replace that SQL statement removing the "on update CURRENT_TIMESTAMP" and it should work for version 4.0.x.
